Session Will Explore a Collaborative Approach to Prospective Risk Adjustment and Care Coordination for Effective Management of Complex Populations
ST. LOUIS — September 18, 2013 — Lumeris, an accountable care delivery innovation company, today announced that Director of Informatics, Dave Corrigan, will deliver a presentation at America’s Health Insurance Plans (AHIP) 2013 Medicare and Medicaid Conference. The presentation, Collaborative Risk Adjustment and Care Coordination for Medicare Advantage and Dual Eligibles, will take place during the Medicaid Conference breakfast session at 7:30 a.m. ET on September 24.
Held September 22-26 at the Renaissance Washington Hotel in Washington, D.C., the AHIP 2013 Medicare and Medicaid Conference combines three conferences into a single event that explores key policy and operations topics critical for policymakers and health insurance industry members working in Medicare and Medicaid.
What: Breakfast Session: Collaborative Risk Adjustment and Care Coordination for Medicare Advantage and Dual Eligibles
When: Tuesday, September 24, at 7:30 a.m. ET
Who: Dave Corrigan, director of informatics
Where: Meeting Room 15, Renaissance Washington Hotel in Washington, D.C.
In this session, Corrigan addresses how taking a collaborative approach to care coordination and risk adjustment to treat patients with multiple chronic conditions can produce outcomes such as closed gaps in care, better management of chronic conditions and more accurate documentation and coding. Corrigan also shows how emerging technology makes this possible through streamlined data and incentives allowing payers to work collaboratively with physicians to deliver a more meaningful, effective patient encounter and ultimately achieve the Triple Aim Plus One: Improved quality, reduced spending and patient and physician satisfaction.
“Collaborative risk adjustment and care coordination is an extremely timely discussion for payers and providers to be having as more Americans are living longer with multiple chronic conditions,” said Corrigan. “On the heels of the eighth annual Healthcare IT Week, this conference is a great venue to facilitate and continue conversations with physicians and payers of lessons learned and the next steps to manage patients and their populations as health care moves from volume- to value-based care. Such collaboration supports the Triple Aim and also produces the provider and patient satisfaction required to truly achieve it.”
During AHIP’s 2013 Medicare and Medicaid Conference, September 22-26, Lumeris representatives are available to discuss accountable care delivery, recent company news and industry trends. Lumeris will be available at Table eight during the conference.
