Value-based Initiative Helps Deliver Quality Care at Lower Cost for Northeast Philadelphia
PHILADELPHIA AND ST. LOUIS — February 19, 2014 — Aria Health and Lumeris today announced a value-based initiative to improve the quality of care for nearly 20,000 Aria Health patients in Northeast Philadelphia by optimizing Aria Health’s performance on Independence Blue Cross’ (IBC) primary and accountable care payment models. The initiative supports Aria Health’s community-based approach to population health and maximizes its investment in nearly 200 employed and affiliated physicians through clinical integration arrangements at the health system’s three hospital campuses, three emergency departments, two outpatient centers, and multiple independent practice locations. These physicians will utilize Lumeris’ technology-enabled solutions to make better value-based decisions that lead to quality care at lower cost, as well as enable greater continuity of care, transition management and care management.

As part of the initiative, Lumeris’ Accountable Delivery System Platform (ADSP)® will aggregate and interpret claims data from the health insurer and clinical data from the health system’s electronic medical records, giving Aria Health and its providers an unprecedented, comprehensive picture of the care its patients are receiving across the care continuum. Using this information, Lumeris will help Aria Health fully realize opportunities for incentives under IBC’s innovative accountable care model called the Integrated Provider Performance Incentive Plan (IPPIP), and their primary care-focused Quality Incentive Payment System (QIPS), which rewards doctors and hospitals for delivering quality, coordinated and efficient care.
