Selects Lumeris technology platform, services to help manage patient health, reduce costs

DENVER — Feb. 13, 2013 — SCL Health System, a faith-based, not-for-profit health care organization, announced today the launch of an accountable care strategy designed to improve the quality of patient care and lower costs at SCL Health System hospitals and clinics. Under this initiative, SCL Health System has selected Lumeris to provide its transformative platform and services solution for accountable care.

Lumeris’ accountable care experts will help SCL Health System fully develop its accountable care strategy, identify appropriate physician partners and select initial patient populations for value-based care initiatives. For example, the strategy will address the unique needs of patients with complex, chronic conditions like cardiovascular disease and diabetes, who often require more robust clinical management to identify gaps in care, anticipate possible complications and ensure appropriate treatment. SCL Health System and Lumeris will continually evaluate the strategy’s outcomes, with the goal of expanding the program to additional patient populations.

In addition, SCL Health System will provide physicians and staff access to Lumeris’ Accountable Delivery System Platform (ADSP)®, which aggregates and analyzes data from across the continuum of care — including medical claims, electronic medical records encounter data, and lab and pharmacy data — to provide a comprehensive view of patient care. The system then sends actionable clinical and financial intelligence to physicians and other stakeholders at the point of medical decision-making. This enables timely value-based health care decisions proven to improve quality of care, reduce costs, increase collaboration and coordination, and improve patient and physician satisfaction.
